Monroe Clinic's diabetes care team believes in the team management approach to diabetes. The care team includes your primary care provider, endocrinologist, registered dietitian, nurse practitioner and diabetes educators. But the most important member of the team is you.
Diabetes Self-Management Education Program
Recognized by the American Diabetes Association for Quality Self-Management Education, the diabetes self-management education program is designed for those who were recently diagnosed with diabetes or who are new to diabetes self-management. Continuing in-depth education is also available for those who have a history of diabetes.
Diabetes Support Group
The diabetes support group provides information on many aspects of living with diabetes. Attendees share their experiences with others who are living with diabetes during this monthly group meeting.
