Prescription Refill

When your doctor writes a prescription, he or she will designate the number of refills the pharmacy can give you.

A prescription refill is a request for more medication(s) under your original prescription that the pharmacy can prepare for you without needing to contact your doctor.

Simply complete the form, located on the right, to request a medication refill from Monroe Clinic pharmacy. Enter a 10-digit prescription number --include the zeroes behind the dash. (Note: Make sure the phone number you put in the form includes your area code.) 

To request multiple prescriptions click "Add Rx number"

NOTE: If you do not use the Monroe Clinic Pharmacy you will need to contact your pharmacy of choice for refills.

Prescription Renewal

When all of your refills have been used (indicated on your prescription bottle when the Refill number is zero) you need a new prescription renewal. The form below will not work for prescription renewals. 

Contact your doctor by using MyChart or call their clinic office.  Many times your doctor can renew your prescription without an office visit; but, in some situations, you may need to make an appointment with your doctor before a medication can be renewed.
