Family Record Access
Monroe Clinic also offers proxy access so you can help manage the health of your family members. Whether it's your parents or your kids, we've got you covered! (Note: Information available through proxy access of children ages 12-17 is limited by law). Because confidentiality is our priority, patients seeking access to family member records must complete the necessary proxy forms, requiring signatures. For more information, call 608-324-3742.

If you've already registered for MyChart, you can download the MyChart app through the Apple App Store or the Android Market. This app lets you do nearly everything MyChart does, but it's advailable on your mobile device - meaning your health information is always at your fingertips.

Is MyChart Secure? Working with an industry leader in electronic medical records for over a decade, Monroe Clinic has taken every precaution to ensure electronic health records are secure and confidential.
Who else can access my health information? Only you and individuals you personally authorize, through the family access option, can obtain your health information.
How much does MyChart cost? it is offered free-of-charge to every Monroe Clinic patient.

URGENT MEDICAL MATTERS
MyChart is NOT to be used for urgent needs. If you are experiencing a medical emergency (chest pain, shortness of breath, etc.) you should dial 9-1-1 or go to the nearest urgent care clinic or emergency room.
